During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. Every new patient receives a full physical examination, as a basis for a specific diagnosis that will determine your course of care. PATIENT FORMS

You will be emailed a link to your new patient portal, containing all new patient forms, before your first visit.  If you do not have access to a computer please arrive at least fifteen minutes early to fill out your new patient forms.

WHAT TO EXPECT

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Educate yourself on your symptoms by reviewing the content on this website. THE NO SURPRISES ACT

The federal no-surprises act requires that patients with no insurance, or who choose self-pay, be given a good-faith estimate of charges for their care.  If such an individual receives a bill that is substantially in excess of the amount(s) quoted, they can challenge the bill in a patient-provider dispute resolution.  If you are a self-pay patient, please let the office know immediately upon arrival so you can receive a good-faith estimate for your care.

Note: every patient at Cronk Chiropractic receives specific care tailored for their case.  There is no one-size-fits-all care plan, and a physical exam will have to be completed before a care plan can be created for your situation.
